Agreement Highlights
Through this collaboration, U.S. Renal Care has adopted a suite of digitally enabled Vantive peritoneal dialysis (PD) products and advanced support services designed to strengthen patient care, streamline clinical workflows, and improve the home dialysis experience:
- Sharesource Connectivity Platform
A secure, two-way remote patient management system that allows U.S. Renal Care clinicians to monitor and act on patients’ treatment data throughout therapy. Using Sharesource, care teams can remotely personalize automated peritoneal dialysis (APD) therapy performed overnight while a patient sleeps and adjust prescriptions as needed to support optimal outcomes.
- Electronic Medical Record (EMR) Integration
Key treatment data from the Sharesource Connectivity Platform flows directly into U.S. Renal Care’s EMR system, giving care teams greater visibility into patients’ therapy. This integration provides data-driven insights to inform daily treatment decisions and supports the ongoing evolution of U.S. Renal Care’s home dialysis care model.
- PD Telecare Program
A proactive remote monitoring and support service for patients using Sharesource-enabled APD cyclers. During the critical first 90 days of therapy and beyond, Vantive’s dedicated PD Telecare nurses review treatment data, alerts, and trends to identify training needs or potential barriers to success. When technical issues arise, PD Telecare support representatives proactively reach out to patients, providing timely coaching and reinforcing techniques to help improve confidence, adherence, and long-term outcomes.
